JOB TITLE: Case Manager: Columbus County (Whiteville)

HOURS: 40 hours/week, Full Time, M-F

JOB DUTIES / RESPONSABILITIES:

The Case Manager (CM) will provide recovery-based support for Coastal Horizons Flourishing Families Program in Columbus County to assist individuals engaged in treatment services in pursuing and achieving health, wellness & recovery goals. The Flourishing Families Program is designed to assist parenting mothers with a substance use disorder and her identified supports. The Case Manager is an integral part of Coastal Horizons Center’s team approach to integrated care for behavioral health and medical care. The CM works with clients referred for this program in accessing and utilizing treatment, mental health, and health care resources due to their addiction, pregnancy and sometimes trauma. Their role is to improve positive client outcomes through linking clients to medication management, coordinating care, educating clients, positive role modeling, building trust between clients and their clinical and medical practitioners, supporting and connecting them to community recovery programs, and enhancing communication and the continuity of care. They will teach, mentor and monitor clients on issues relevant to their care.

The Case Manager will also facilitate group classes and related services (to clients and family members) and provide appropriate documentation in the client record. As a member of a multidisciplinary team, they consult with other corresponding care team members to coordinate the services of patient education, preventive care and recovery management.

REQUIRED EDUCATIONAL/EXPERIENTIAL QUALIFICATIONS:

Bachelor’s Degree in Psychology, Social Work, or other related social service program. Experience in working with those in addiction is beneficial.

SALARY & BENEFITS: Commensurate with experience with a range of $36,000-40,000, medical/dental insurance, life insurance, LTD disability insurance, retirement savings plan/401K, paid time off programs.
