JOB TITLE: Healthy Opportunities Program (HOP) Care Manager
HOURS: Full time, Non-Exempt
Job Duties / Responsibilities:
The Healthy Opportunities Program (HOP) in North Carolina will cover the cost of evidence-based non-medical approved services related to housing, food, transportation, interpersonal safety and toxic stress that directly impact people’s health outcomes and health care costs. HOP services open to qualifying Medicaid enrollees. Coastal Horizons was selected to be a Human Service Organization (HSO) for Brunswick, New Hanover, and Pender counties.
The HOP Care Manager provides direct support to the client addressing identified social determinants of health (SDOH) needs so clients may achieve better health outcomes. The HOP Care Manager will be responsible for managing incoming referrals through NCCare360 platform/Unite Us. Provide care coordination activities, supporting and connecting clients referred for assistance in areas of housing, transportation, and food insecurity. HOP Care Manager will interact with all members of the healthcare team (internal and external) to keep the lines of communication open. The HOP Care Manager will provide appropriate documentation in the client record and NCCare360. Expectation to attend required trainings, quality improvement activities, community meetings related to the Healthy Opportunity Program.
Qualifications / Skills:
The Care Manager has a bachelor’s degree in a human service, health related field, or similar degree/experience. Preferred, two year’s experience providing care management, case management, or care coordination to the population being served. Experience working with those with behavioral health needs is beneficial. Computer proficiency with Microsoft Word, Excel, etc.
